Create a form field
1.Choose Tools > Advanced Editing > Forms > Show Forms Toolbar.
2.On the Forms toolbar, select a forms tool and do one of the following:
ΓÇóDrag the cross-hair pointer to create a form field in the desired size.
ΓÇóDouble-click on the page to create a form field at that location using the default size.
3.Specify the properties for the form field in the Properties dialog box.
Note:  The Properties dialog box can also be opened by double-clicking the field using the same form field tool or the Select Object tool .
